FIRST VISIT:

ARRIVE 15 MINUTES EARLY
TO COMPLETE REQUIRED PAPERWORK

We will make every possible effort to make your 1st visit very comfortable. Bring whatever questions or concerns you have so that our doctor can answer them for you.

Health History Intake: At Free Spirit Chiropractic, we will evaluate your health history. Be prepared to answer questions and have dates/ages available from birth until now concerning the following: past injuries, slips/falls, auto accidents, work related injuries, athletic injuries, surgeries, chronic problems and your current symptoms.

You will be asked: Where are your sensitive or painful areas? Is your pain constant or does it come and go? When did the pain start? Was there a particular incident that caused the pain? Is your pain progressively getting worse or is it staying the same? Have you had this pain before?

What makes the pain worse? What makes the pain better? Have you received any other care/treatment regarding this pain (medical, chiropractic, massage, acupuncture, physical therapy...)? Has it helped?

In addition, the doctor will ask you about your lifestyle habits (diet, water intake, smoking/drinking habits, job/work, exercise, etc.).

Bring a list of your current vitamins, supplements and herbs you are taking. If you are taking medications, bring a complete list to include: name of the medication, dosage, reason for taking, the doctor who prescribed and how long you have been taking the medication.

Chiropractic Examination: You will then receive a thorough examination. We will examine your posture, your spine, your muscles and your overall neurology and biomechanics of how your body is working right now.

That completes your 1st visit.

2nd VISIT:

Report of Findings/Chiropractic Care Plan: The doctor will review the findings of your health history and chiropractic examination prior to your 2nd visit.

On your 2nd visit, the most effective personalized chiropractic care plan will be recommended. Be sure to ask as many questions as you need to in order to fully understand your condition and the recommendations being made.

Your doctor will let you know if chiropractic is the best choice for your present condition during the Report of Findings.

A personalized care plan will be designed for you with your specific health challenges, personal concerns and goals in mind. This plan will provide the most effective way to address your painful condition and return you to your Ultimate Wellness Capacity ™.

In addition to chiropractic adjustments, other things may be recommended to further enhance and ensure your speedy and complete recovery (exercises, dietary recommendations, nutrition supplements, massage, physical therapy, acupuncture, etc.).

1st Adjustment: You will receive your 1st adjustment on the 2nd visit. Following your 1st adjustment, do not plan any strenuous workouts or activities that day. It takes 24-48 hours for your body to integrate all of the neurological and mechanical changes that take place.

Regular workouts and activities can be in place beyond the 1st adjustment/1st day (unless your condition prevents you from doing that).
